Files Not Getting Overwritten when set to Always

We are using InstallShield 2020 R3 SP1. Installations of Basic MSI builds using this product has been demonstrating failures to overwrite existing files with ones in the build despite these files being set to Always Overwrite.

This has not occurred with all the installations built with this version we have done so far, but it is occurring frequently enough to be of a concern.  We have not been able to identify a causative factor.  It does not appear to be related to the version of Windows that the build is installed on.  It may be related to which version of InstallShield was used for the prior build that was installed on the system.

Is this a known problem with this version of InstallShield? Is there a fix?
