what does this icon mean?

What does this icon mean in the Files and Folders view?

Its a red explanation mark on top of the component / folder icon.

It means that the component is not contained in any features, which means it will never be installed; you can associate the component with a feature by right-clicking a feature in the Setup Design view...

you can also associate it with a feature by right clicking the component on the file and folders screen you have shown and selecting the properties option, then going to the features tab.
