How do you create lm_new_md.obj and what does it contain?

You create the _md versions of libraries (including lm_new_md.obj) by running the make file with MD=1 (nmake MD=1, for example); searching the help document devenv.pdf (Creating a Licensed Product...) for "lm_new" and "MD" will give more information.
